$12 Per Month - This membership provides all of the Basic Discounts and services available to you and your entire family.

Gold Membership

$1,000 Accident Medical Coverage
Individual/Family - $19.95

The Gold Membership includes all the Discounts, Benefits and Services found in the Basic Membership with Group Accident Medical Protection up to $1,000 per accident.

Platinum Membership

$2,500 Accident Medical Coverage
Individual/Family - $29.95

The Platinum Membership includes all the Discounts, Benefits and Services found in the Basic Membership with Group Accident Medical Protection up to $2,500 per accident.

You could have an accident at any moment - while driving to work, doing chores around the house, exercising, or just playing with the kids.· Accident Medical Insurance provides benefits for medical treatment if you're injured in an accident.

There is no dedicated provider network so you are NOT required to go to a particular Doctor or Hospital for treatment.· This plan will pay you or your treatment provider for expenses resulting from an accident.

Guarantee Trust Life Insurance Company (GTL) provides the Accident Medical coverage and Accidental Death and Dismemberment coverage. GTL does not provide nor is affiliated with the discount programs provided as part of membership in Consumer Benefits of America (CBA)
